About Northeast Concierge Medicine

Northeast Concierge Medicine is a membership-based concierge primary care practice for adults, located at 24 South Main Street in Putnam, Connecticut. Dr. Joseph Botta founded the practice on January 1, 2009, and it has served northeast Connecticut and the surrounding region ever since.

Our Mission: To Always Be Personalized & Patient – Because We Truly Care

Our providers chose the concierge model of medicine because they are dedicated to treating people well, and having the time to do it. Each of them recognized earlier in their careers that the fast appointments and high volume, low touch experience typical of traditional practices is not designed for that, and they wanted to do better by their patients.

That’s exactly what drove Dr. Joseph Botta, MD to found the practice in January, 2009. At the time, others told him it wouldn’t work. Nearly 20 years later, our practice is providing top-quality care to patients all across the Northeast Connecticut region and nearby Massachusetts and Rhode Island.

How our concierge medicine model works

Our model is a hybrid. We bill your health insurance for your visits, exactly as any primary care practice does, and you pay your deductibles, copays, and coinsurance as usual.

Separately, you pay a monthly membership fee to the practice, which is equal to about the cost of going out to dinner once per month. This allows our practice to keep doing everything for you that insurance stopped covering — the time, the access, the comprehensive coordination, and the personalized attention.

Specifically, your membership pays for:

  • Time. A minimum thirty-minute appointment, an hour or more for a physical, and a new patient visit that runs ninety minutes between staff and physician.
  • Access. Your physician’s direct cell number for nights, weekends, and holidays. No answering service.
  • A person on the phone. Our staff answers calls directly. There is no phone tree and no automated queue.
  • Same-day responsiveness. Same-day or next-day appointments when you’re sick. Same-day prescription refills and call-backs, often within the hour.
  • Coordination. Referrals, prior authorizations, and diagnostic scheduling handled by our office, not by you.
  • An unrushed panel. We deliberately limit how many patients each physician carries. That limit is the product.
  • All the services and care you see listed on this website, whenever you need them.

What a first visit looks like

  1. Call us. We’ll explain the membership model, confirm your insurance, and answer your questions. No pressure, no sales script.
  2. Enroll. Paperwork, membership agreement, and payment; we will then request your records from your previous physician.
  3. Your first visit. About ninety minutes total — thirty with our clinical staff, an hour with your physician. We take a full history: your conditions, medications, screenings, family history, and how you actually live.
  4. Your plan. You leave understanding what happens next, with your physician’s contact information in hand.

Meet our team

Four clinicians care for our patients. Each carries their own panel, so you see your own physician — with easy coverage between them when you’re sick and yours is away.

And the rest of the team

Nurses, medical assistants, front-desk staff, and an office manager — all local, most with decades of clinical experience. They know your voice on the phone, and often your spouse’s and your daughter’s too. Some callers don’t say their name because they don’t have to. Also: Luna, Dr. Botta’s dog, is here most days. Her practice specialty is inducing smiles.

Our Office

Our office doesn’t look like a typical doctor’s office, and that’s on purpose.

Wood throughout, warm light, and a different theme for each exam room make it feel inviting and not sterile. Dr. Botta’s fish tanks add even more interest — including one his father gave him when he made the honor roll at sixteen, which he’s kept for forty years.

    Rooted in this community

Putnam is a small town that behaves like a bigger one. People here are invested in each other, and this practice is part of that — through Day Kimball Hospital, the local nursing facilities we round at, and seventeen years of patients who’ve been with us since the beginning.

Northeast Connecticut is short on physicians. Practices have closed and physicians have retired, leaving thousands of people in this region without primary care. We’re growing carefully to meet part of that need, without growing past the point where this model still works.

We also want to acknowledge Dr. Ronald Klare, who retires from clinical practice on December 31, 2026 after nearly fifty years of caring for families across this region — first alongside his father, Dr. Rudy Klare, at Woodstock Medical Group, and in recent years with us. His patients are transitioning to Dr. Wolslegel with his personal involvement in every handoff.

    FAQ

How much does concierge medicine cost at Northeast Concierge Medicine?

Our membership is a monthly fee, roughly the cost of going out to dinner once a month. It is charged separately from your insurance and covers the time and access described above, not the medical care itself, which is billed to your insurance. Call (860) 315-9026 for the current fee and payment plan options.

Do I still need health insurance?

Yes. Northeast Concierge Medicine bills your health insurance for office visits, and you remain responsible for your usual copays and deductibles. The membership fee is separate from insurance and does not replace health coverage.

    What insurance do you accept?
We accept Blue Cross/Blue Shield, Cigna, Aetna, United Healthcare PPO, and Harvard Pilgrim, along with Traditional Medicare and select Medicare Advantage plans from Blue Cross/Blue Shield, United Healthcare PPO, and Aetna. Call to confirm your specific plan.
Is the membership fee tax-deductible or payable from an HSA?
The membership fee is a value-added service contract, separate from health insurance, and is generally not tax-deductible. Talk to your tax advisor about your particular situation.
How is concierge medicine different from direct primary care?
Direct primary care practices typically do not bill insurance at all; patients pay a membership fee that covers their care. Northeast Concierge Medicine uses a hybrid concierge model: we bill your insurance for visits as a traditional practice does, and the membership fee covers time, access, and coordination on top of that.
    Do you treat children?
No. Northeast Concierge Medicine is an adult primary care practice.
How many patients does each physician see?
Far fewer than a traditional practice. Panel size is deliberately limited so that appointments can run thirty minutes or longer without overbooking. A traditional primary care physician may be asked to see twenty-five or more patients a day; that is the model this practice was founded to avoid.
What happens if I need care after hours?
Call your physician’s cell phone directly. Nights, weekends, and holidays, you reach Dr. Botta or Dr. Candow — not an answering service. For a true emergency, call 911.
