Patient Resources

Everything you need before, between, and after your visits — whether you’re already part of the practice or considering joining.

Existing Patient Portal

An online patient portal is available to those patients who would like it. The portal is a convenient way to securely communicate about non-urgent matters with your physician and care team. Upon request, you may also have access to view your records, such as lab results. For anything that needs an immediate response — a new symptom, a medication problem, a question about a hospital discharge — please call.

To set up your patient portal, please call us at (860) 315-9026.

Insurance and billing

We bill your health insurance for office visits, and you’re responsible for your usual copay and deductible. Your monthly membership fee is separate from insurance and covers the time, access, and coordination that insurance doesn’t pay for.

Commercial plans accepted:

  • Blue Cross/Blue Shield
  • Aetna
  • United Healthcare (PPO)

Medicare:

  • Traditional Medicare
  • Select Medicare Advantage plans from Blue Cross/Blue Shield, United Healthcare (PPO), and Aetna

Self- Pay

For those patients who have a non-participating insurance, we offer an affordable self-pay option.

New Patients

Dr. Mae Wolslegel is accepting new patients now. Becoming a patient here starts with a phone call, not a form.

Call (860) 315-9026 and we’ll explain how the membership works, tell you the current fee, confirm your insurance, and answer your questions. If it’s a fit, enrollment takes one conversation.

What to bring to your first visit

  • Your insurance card and photo ID
  • A complete list of your current medications, including supplements and anything over the counter — the bottles themselves are even better
  • The names and contact information of any specialists you see
  • Records or test results from your previous physician, if you have them (we’ll also request them directly)
  • Your questions. Write them down. There’s time for all of them.

Plan on about ninety minutes: thirty with our clinical team, an hour with your physician.

FAQ

How do I get my prescription refilled?

Call the office at (860) 315-9026. Prescription refills are handled the same day, and often within the hour.

What if I need my physician on a weekend?

Call your physician’s cell phone directly. Northeast Concierge Medicine patients reach Dr. Botta or Dr. Candow on nights, weekends, and holidays — there is no answering service between you and your physician.

Can I get my medical records?

Yes. Lab results and visit summaries are available in your patient portal, and the office can provide your full record on request. Call (860) 315-9026.

How do I transfer my records from my previous physician?

We request them for you as part of enrollment. Bringing whatever you already have to your first visit helps us start faster.
