Cardiovascular Care

Cardiovascular care at Northeast Concierge Medicine means your blood pressure, cholesterol, and cardiac risk are managed continuously by your own physician — not reviewed once a year and adjusted by whoever is on the schedule.

Heart disease is the leading cause of death in the United States, and most of what determines its course happens between appointments. That’s the part traditional primary care can’t reach. Our membership model exists to close it.

What cardiovascular care includes

  • Blood pressure evaluation and medication management, with recheck visits when anything changes
  • If you are experiencing a non-emergency and unexpected side effect of your medication, simply call the office and we will help.
  • Cholesterol and lipid management, including statin decisions made with you rather than for you
  • Cardiac risk assessment factoring in family history, diabetes, kidney function, and lifestyle
  • Ongoing management of heart failure, atrial fibrillation, and coronary artery disease alongside your cardiologist
  • EKG testing
  • Post-hospitalization follow-up after a cardiac event, admission, or procedure
  • Referral, scheduling, and coordination with regional cardiology
A blood pressure medication that isn’t working needs a follow-up in two weeks, not four months. A patient who stopped taking a statin because of muscle aches needs a conversation, not a refill reminder. Neither fits in a ten-minute visit. Here, the follow-up happens because there’s room in the schedule for it. If your numbers move in the wrong direction, we see you again. If you have a question about a side effect on a Sunday, you call your physician’s cell.

Working with your cardiologist

We coordinate directly with cardiology practices across northeast Connecticut, eastern Massachusetts, and Rhode Island, including tertiary centers when a case calls for it. Your physician makes the referral, sends the records, and follows up on the report — then explains what it means in plain language.

FAQ

Do I still need a cardiologist?

Often, yes. Northeast Concierge Medicine provides cardiovascular care within primary care — daily management, medication adjustment, risk reduction, and follow-up — and refers to cardiology for procedures, advanced imaging, and specialist evaluation. We coordinate the referral and follow up on the results.

Can you manage my blood pressure medication?
Yes. Blood pressure evaluation and medication management is core primary care work, and our appointments are long enough to do it properly, with recheck visits scheduled whenever a medication changes.
What happens after a heart attack or cardiac procedure?
We provide post-hospitalization follow-up, reconcile every medication change made during your admission, coordinate with your cardiologist, and see you soon after discharge — the window when most avoidable readmissions happen.
